Scope and Contents Crutch's Pharmacy Prescription Book (2012-08-0) contains 8-10 prescription forms per page (glued on), beginning with prescription #142995/December 17, 1913 and ending with #146624/December 3, 1914. No patient names on prescription forms, and only half of the prescriptions are on letterhead. Possible Litchfield doctors included in this book are, Charles I. Page, Marvin Z.
